If you require a W9 form to submit payment, please contact Emily Clifford, info@mass-pa.com. Please complete all necessary paper work in this packet to reserve your booth or register online.
CANCELLATION
In the event of a cancellation, notice must be received, in writing, two (2) weeks before the event (March 7) to receive a full refund minus a $50.00 per booth administrative fee. Cancellations made after March 7 will not be ineligible for a refund.
